Ketosis is the process by which the body, suffering low intake of carbohydrates,  instead converts stored fat into energy. During this conversion, ketones are produced as a by-product, causing an unpleasant fruity breath odor that may be mistaken for alcohol. Prolonged severe ketosis can change the acidity of the blood, and may eventually lead to serious damage to the liver and kidneys. Additional information.
