Continuum of Care [continuity of care]

Continuum of care [continuity of care] refers to ongoing process of treatment and recovery following initial treatment intervention. For example: if patient is treated at the residential level of care, aftercare or continuum of care treatment planning is essential prior to discharge which may include outpatient treatment with adolescent medicine specialist, Maudsley therapist, dietitian and psychiatrist. This also refers to continuing with treatment team providers throughout the phases/stages of treatment to full medical and cognitive recovery, and relapse prevention.
