Treatment Team Collaboration

Treatment Team Collaboration refers to cooperation and communication among members of the treatment team. It is based on the idea that when the team communicates and is on the same page they will then be able to send a unified message to the family and the patient, helping prevent confusion and miscommunication.  If there is a change in providers, collaboration extends between new and former providers. Parents may need to sign consent forms/releases of information that authorize current and former treatment providers to share information, discuss treatment and progress attained to date as well as continuum of care treatment recommendations.
